Non-immigrants who are allowed to stay in the United States for a certain period of time and who are not permitted to work in the United States will have to apply for and obtain employment authorization documents, to work in the country as non-immigrants. This does not apply to naturalized US citizens and Green Card holders and they need not obtain work permits to work in America. You may electronically file an application for an employment authorization document or you can also print the application and complete it. However, you can complete your application on your computer and then print it. You need to complete all the 17 questions and almost all the questions are self-explanatory. If you are filing the paper version of the form, you will have to mail your petition to the right lockbox facility and the filing address depends on your eligibility category.
You can e-file Form I-765, if you are a refugee or an asylee. Citizens of Palau and Micronesia/Marshall Islands, foreign students and family-based non-immigrant (K-1 or K-2) visa holders, may file this petition online. People who are applying for fee waivers, DACA applicants, employment-based non-immigrant visa holders, K-3 and K-4 visa holders, dependents of A-1, A-2, G-1, G-3 and G-4 visa holders cannot file this petition online and they will have to file the paper version of the form. If you e-file Form I-765, you will have to mail the supporting documents to the address that is printed on the confirmation receipt that you receive after e-filing your petition. You can pay the filing fee online, while e-filing your petition and if you are filing the paper version of the form, you will have to send money orders or checks and the form filing fee is $380.

You will have to write your name that is written on your passport and your middle and your family names and must leave spaces in between your family, first and your middle names.

If you use some other names, other than the name that is written on your passport, you will have to enter the other names here and you must also write your maiden name.

Write your address in the space provided below this question. If you live in an apartment, write the apartment number, the name of your town, state and the zip code, in the respective spaces. Write your correct address and remember that your EAD will be mailed to the address that you provide.
Country of Citizenship/Nationality
You will have to write the name of your home country.
Place of Birth
Your place of birth is the country where you were born.
Date of Birth
Write your date of birth in the format that is mentioned beside this question – month/day/year.
Gender
You will have to check the relevant box and if you are a male, check the box beside male, if not check the box beside female.

If you have already applied for an EAD, you will have to write the name of the USCIS office where you applied for an EAD. If you already hold an EAD, you will have to submit a copy of your work permit along with Form I-765. If you had applied for an EAD and if you were not issued one, you will have to attach a copy of the denial notice.
